Seeking Clinics for SAFEST Choice

Proof Alliance is seeking community health centers (CHC) in the Upper Midwest that are interested in participating in the SAFEST Choice Learning Collaborative, a virtual ECHO group focused on fetal alcohol spectrum disorders (FASD). CHCs are invited to participate in Prenatal and/or Pediatric cohorts to learn FASD specialty-specific core competencies. The prenatal cohort will learn how to screen for and counsel women about the risks of alcohol use during pregnancy, and the pediatric cohort will learn how to identify and care for children with FASD. Participating CHCs will receive free continuing education and ongoing technical assistance over the course of a year, as well as a stipend to support a champion at their site. Reflecting on the History of Health Care Homes!

Being a nurse with experience in public health, maternal/child health, care coordination, and behavioral health, I know the importance of primary care. In an effort to promote primary care, I joined the MDH Health Care Homes (HCH) team over 8 years ago and have been a member of the PCC CoP Advisory Group since inception. 

The HCH or “patient centered medical home” idea originated in 1967 when the American Academy of Pediatrics (AAP) introduced the medical home concept to coordinate the care of children with chronic conditions. In 2002, the AAP expanded the medical home concept to include the following operational characteristics: accessible, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective care. By 2005, the American College of Physicians (ACP) had developed an advanced medical home model for all ages, which involved the use of evidence-based medicine, clinical decision-support tools, the Chronic Care Model, care plans, “enhanced and convenient” access to care, quantitative indicators of quality, HIT, and performance feedback. 

MDH HCH certification was crafted from those concepts; the idea is that a clinician/clinic provides its patients a home base for health care and helps them navigate the health care system, they are a patient’s “health care home.” The goals of the HCH model are to continue building a strong primary care foundation to improve healthcare quality, value of care, and enhance the experience of both patients and clinic staff. HCH clinics look very different throughout Minnesota, but all provide care coordination and collaboration between primary care clinicians and community resources to support whole person care and facilitate the broader goals of improving population health and health equity.

And to think, this all started with the pediatric patient with special health needs over 50 years ago.
