Pediatric Care Coordination:

Community of Practice

Tuesday, July 20, 2021

Greetings,

Summer is here! In Minnesota, that means 90-degree heat one day, and then grabbing a sweatshirt the next. We’re excited to share Community of Practice updates below. As always, thank you for being part of strengthening this community! You drive the Community of Practice by submitting events to add to our calendar, attending quarterly webinars, sharing your feedback through our recent member survey, and more. Thank you for your participation!! If you know of other people who may be interested in being part of this community, please invite them to become a free member.

Want your work featured in an animated video?

We want to highlight how people who manage care for children and youth with special needs (CYSHN) have helped kids and their families navigate challenges related to COVID-19. Working with an artist, we'll share these stories through short, animated videos. If you want your work featured in an animated video, fill out this short form, and we’ll be in touch!

Interactive virtual discussion series

Last week, we heard from MDH’s Dr. Chomilo and Gillette Children’s Specialty Healthcare’s Tina Schlottman and Britta Saucerman during the Let’s Talk discussion, “Understanding Covid-19 Vaccines and Concerns for Children with Specialized Needs and Disabilities.” Stay tuned for key takeaways and resources from this discussion!

Thanks for your feedback!

A big thank you to everyone who shared their feedback about the Community of Practice through the member survey. Two lucky raffle winners each took home a handsome YETI mug!! Look for survey highlights in a future eNews.

The Chloe Barnes Advisory Council on Rare Diseases seeks input

For primary care providers, nurse practitioners, and physician assistants: The Chloe Barnes Advisory Council on Rare Diseases needs your input on a 5 minute survey. There are many barriers to diagnosis and care in the rare disease community and the goal of the survey is to better understand the medical community's point of view and issues encountered when treating a rare disease patient. The goal is to improve care for the 1 in 10 Minnesotans living with a rare disease. Please consider supporting this vital effort to address the needs of an often-overlooked patient population. If you’re not a primary care physician, physician assistant, or nurse practitioner, we invite you to share this link. You can also contact the Council at mnrdac@umn.edu with questions.
