Pediatric Care Coordination:

Community of Practice

Tuesday, June 7, 2022

Greetings,

It was wonderful to be with so many of you on May 12 for our virtual statewide conference, Caring for ourselves, our network and our kids: Laying the foundation for meaningful connection.

In this newsletter, you’ll find:

  • Statewide conference highlights, including a list of resources shared throughout the day and a poem that uniquely sums up the conference

  • Information on how to navigate the baby formula shortage

  • A sneak peak at the September Community of Practice webinar

  • The Community of Practice in the news!

We also invite you to regularly visit our website, http://www.mnpedcares.com to learn about relevant resources, upcoming events, and other information that will help you support kids and their families. For example, do you know about our Community Organizations page? This directory lists organizations and services available across Minnesota, searchable by location and topic. For example, if you work with a family in Duluth, you can zoom in to Duluth on the map to find local organizations. Or, you can click the Search button to search by geography or by category, such as behavioral health.

Have a community organization that you’d like us to add? Great! Let us know about it here!

As always, thanks for your participation in the Community of Practice. You make our work possible!

All our best,
Wendy Berghorst and the ACET team

Statewide conference highlights

Thank you to everyone who made our conference a success! Over 100 people gathered virtually on May 12 to learn, share, and network.

What some conference participants said about their experiences at the conference…

  • I was inspired by the fact that we were all individuals from across the entire state of MN with a collective goal to help our pediatric population in multiple modalities and avenues, but with the same end goal of helping children be successful. It was useful to hear from other professionals within the pediatric care profession and what they are doing to best support their patients.

  • It was nice to hear that other professionals experience similar challenges with care and access, and was nice to share resources.

  • It inspired me to keep reaching out and connecting with community partners to better serve individuals in need.

Artist Lindsey Samples listened throughout the day and assembled a poem based on what she heard. The words come directly from conference participants. Scroll to the bottom of this email to read the poem!

Finally, we assembled a list of resources shared throughout the day by panelists, speakers, and conference participants. Find it here!

Caring for ourselves, our network and our kids: Laying the foundation for meaningful connection
Conference Poem

Whether dark grey like a heavy storm

Or sunny like a daffodil

We show up for community care

Integrating systems level change

To disrupt disparity

We may not solve every problem

But we can guide toward solutions

Approach the work with a beginner’s mind

Letting go of assumptions

Cultural humility means becoming a student of the patient

Their story is unique, heard, and validated

Sensitive, approachable

Letting the individual guide their care

Can you become what you’re looking for?

Be the representation I wish I saw?

I am responsible for my learning

Research, resources, conversations

Mistakes will happen, get comfortable being uncomfortable

Apologize and refocus your lens to learn

Reaching beyond barriers

Participating in planning

Proactive prevention

The long wait to receive services

Delayed by complex, cumbersome systems and staffing shortages

Grasping for the light at the end of the tunnel

Community collaboration is key to opening doors

Let’s stop masking our needs

We are powerful not despite the complexities, but because of them

Accessibility for one is accessibility for all

Simple shifts confront our own biases

It’s ok to rest on this walk towards collective liberation

Our interdependence can be the building blocks for this web of care

Train up and bring the learning back

Whether resiliency, ACES, or making transitions

Let’s source opportunities to recreate

Streamlining engagement together and building relationships

Podding up by sector or geography

Continue to educate families in partnership, planning, and goals

Breaking through the stigmas

Identify early, support appropriately

Open, collaborative, empathetic problem solving for all families.

Envision yourself in this future

We dream.
