Happy New Year from the Pediatric Care Coordination: Community of Practice! Our New Year’s resolution is to expand our community and continue to provide you with educational and networking opportunities. 

In order to reflect on the past year, we are looking for your perspective on our Community of Practice. The noon-time Project ECHO online sessions on developing a sustainable and collaborative knowledge-sharing community to improve the transition from pediatric to adult care for youth with medical complexity will culminate on March 13, with a day-long, in-person summit.

"This summit will offer a combination of experts in transition care, patients, and those just starting out in this very important field," said project medical director Tori Bahr, MD.
The noon-time sessions and summit, made possible by a grant from the COPIC Medical Foundation, aim to improve Minnesota adult primary care clinicians' competence and confidence in managing youth with complex conditions, thereby increasing the capacity and number of willing clinicians to accept new patients.

Participate in the Minnesota Integrated Care for Early Childhood Initiative Survey!

The Minnesota Department of Health is helping to sponsor a community-led initiative in the state called the Minnesota Integrated Care for Early Childhood Initiative. An initial goal of this initiative is to conduct a comprehensive systems assessment focusing on early childhood services for African American families and children – especially related to how early childhood services coordinate with health care. The Systems Assessment will provide insight into what is working well, what is beginning to work more effectively, what gaps remain, and where opportunities exist to address these gaps.

As part of their work, they are interviewing early childhood health professionals to help us better understand the early childhood healthcare system. They are interested in learning more about what your experiences have been with providing services to African American families and children, what challenges and barriers exist, and what could help to improve services for this population.
