Objectives:

  • Learn from local Community Resource Hub navigators who are serving families

Shawn Holmes Bio: Shawn Holmes is the Help Me Connect Coordinator at the MN Department of Health. She leads a cross-agency team from the MN Departments of Health, Education, and Human Services that is responsible for creating and implementing an online, early childhood navigator that connects expectant families and those with young children (birth – 8 years old) with services in their local communities. Shawn previously worked in the Children & Youth with Special Health Needs Section at MDH as the coordinator of the Follow Along Program - a statewide developmental and social-emotional screening program provided through local public health departments for children birth to 36 months who are at risk of health or developmental issues.

Rachael Ossmann Bio: Rachael Ossmann has over 10 years’ experience working within the childcare field as teacher, coordinator, and director. She currently is employed with NorthPoint Health & Wellness Center as the Preschool Development Grant Project Coordinator. Within this role, she has the privilege of bridging communities of likeminded individuals while also strengthening programs within NorthPoint. She is encouraged at the impact PDG has made so far, and excited for its continued growth in the future!

Zane Bail Bio: Zane Bail provides strategic leadership for a portfolio of programs, including the Northland Community Resource Hub.  Zane has more than 25 years of experience in the areas of community organizing, program design, early care and education, intergenerational, and aging, and evaluation.  Zane holds a Master of Arts degree in Urban & Regional Studies and Public Administration from Minnesota State University Mankato.
